We are currently in network with PacificSource, Providence and can submit claims on your behalf.
If we are out of network with your insurance company, you will need to pay for services on or before the date of treatment. You then can access to the necessary documents through the patient portal to submit for reimbursement from your insurance company yourself.
We are not able to accept new patients with Medicare and Medicare Replacement plans at this time.

If you have an insurance company that we are not in network (other than PacificSource, Providence Health) with or if you have a high deductible below is our price list for services.
